The purpose of a community health needs assessment is to identify potential barriers to accessing healthcare, identify ways to prevent illness, and identify social, behavioral, environmental and/or other factors that influence health. As part of the Hebrew Senior Care (HSC) Community Benefits Program, a Community Needs Assessment is periodically completed to ensure that we meet the needs of our local community to promote preventive care and to improve the health status for working families and populations at risk.
